With kind cooperation of the members of Indian Society of Renal and Transplant Pathology (ISRTP), this forum is strengthening day by day. Now the time has come for all of us to contribute and develop a platform for academic exchange. The diagnostic practice of renal pathology is supported by four pillars; histopathology, immunofluorescence, electron microscopy and molecular pathology. In India, majority of the centres/experts depend on the first two techniques for biopsy diagnosis. In most of the cases these may be sufficient to arrive at a conclusion. However there are few cases where electron microscopy and or molecular techniques are needed for confirmation. Considering the economic situation in a developing country like India, it is difficult for some centres even to perform immunofluorescence routinely for all renal biopsies. With the recent advances in LED technology, the price of fluorescence microscope has reduced significantly. Setting up of diagnostic immunofluorescence facility may be easier now. Without immunofluorescence back up, it is impossible to clinch the diagnosis of immune-complex mediated glomerulonephritis like IgA nephropathy. As chronic kidney disease (CKD) is becoming a major challenge in health care system in our country with increasing incidence every year, it is extremely important to diagnose renal diseases as early as possible. The pivotal role of renal pathologists must be emphasized in this situation. There is a paucity of organized training courses for renal pathology in India. We need to strengthen this area. A collaborative effort of the leading centres and institutions, supplemented with help from the international organizations may solve our problem. I appeal to all members of ISRTP as well as others to send their suggestions in this regard. We are in the process of developing a web based discussion group for all ISRTP members and others interested experts from different specialities. This may be of great help to find ways for improvements.
